Documents needed to qualify for a position
All locum physiotherapy candidates must provide the following documents in order to qualify for a position:
- Proof of Hepatitis B immunisation (lab report within the past 5 years)
- Proof of Rubella immunisation (lab report showing positive immunity or evidence of two MMRs)
- Proof of Varicella immunisation (written proof that BCG scar has been sighted by OH Nurse/Doctor or TB skin test under 2 years old)
- Proof of immunity for Measles and Mumps (lab report showing positive immunity or evidence of two MMRs)
- Two professional references from employers within the past 24 months
- Proof of address via two copies of recent utility bills
- A fully completed and signed registration form and Terms of Conditions
- Up-to-date CV (comprehensive without gaps in employment)
- Two passport photos
- Proof of professional body registration (UK-HPC)
- Qualifications and copies of all relevant certificates
- Enhanced DBS check, formerly known as a CRB check
- Police report from country of origin (within three months of arriving in the UK)
- Certificates for manual handling and basic life support (less than 12 months old)
- Copy of a valid passport (either the original document or a verified copy for the CRB)
- Proof of eligibility to work in the United Kingdom (for applicants without a UK or EU passport)
